What Is a Healthcare BPO? The US Medical Billing and Revenue Cycle Process, Step by Step
By JobKarle Editorial Team · Published · 4 min read
How healthcare BPOs support US hospitals and clinics, and a start-to-finish walk through the revenue cycle from patient registration to payment and follow-up.
What a healthcare BPO does
A healthcare BPO is an outsourcing company that handles non-clinical work for hospitals, doctors' practices, labs, and health insurance companies. In India, most of this work supports the United States, where billing and insurance rules are complex and expensive to run in-house.
The work is administrative, not medical treatment. Staff do not diagnose or treat patients. They handle the paperwork and phone calls that make sure providers are paid for the care they gave.
Two sides of the work: provider side and payer side
- Provider side: supports hospitals, clinics and physician groups. Work includes patient registration, eligibility checks, medical coding, billing, payment posting, denial handling and AR (accounts receivable) follow-up. Together this is called revenue cycle management (RCM).
- Payer side: supports health insurance companies, called payers. Work includes claims processing, enrolment, member services, provider data management and benefits configuration, often on health plan platforms such as Facets and QNXT.
The revenue cycle from start to finish
This is the usual path of a single medical bill. Each step is a type of job in a healthcare BPO.
- Scheduling and patient registration: the patient's personal and insurance details are collected when the appointment is booked or the patient arrives.
- Eligibility and benefits verification: staff check with the insurance company that the patient's coverage is active and what it covers, before the visit.
- Prior authorization: for certain treatments or tests, the insurer must approve in advance. Staff request and track this approval.
- The visit happens: the doctor treats the patient and records what was done in the medical record.
- Charge entry: the services provided are entered into the billing system with the correct dates and providers.
- Medical coding: certified coders turn the doctor's notes into standard codes for diagnoses and procedures, so the insurer can understand what was done.
- Claim creation and checking: a claim is built from the coded information and checked for errors before it is sent.
- Claim submission: the claim is sent electronically to the insurer, usually through a clearinghouse that checks the format.
- Adjudication: the insurer reviews the claim and decides whether to pay, reduce or deny it.
- Payment posting: payments and adjustment details arrive from the insurer and are recorded against the claim in the system.
- Denial management: if the claim was denied or underpaid, staff find the reason, fix the problem and appeal or resubmit.
- AR follow-up: for claims that are unpaid or still pending, AR callers contact the insurer to find out the status and get the claim moving.
- Patient billing: the remaining balance that the patient owes, such as a copay or deductible, is billed and collected.
Why this work is outsourced to India
US providers use outsourcing to lower costs, handle peaks in claim volume, and get coverage across US working hours. India has a large pool of English-speaking graduates, and training for these roles is widely available. This is why many healthcare BPO jobs here run on night shifts.
Who works in healthcare BPOs
Roles range from freshers in AR calling and data entry, to certified coders and quality analysts, to managers who run client relationships. Some roles need a graduate degree or a life-science or commerce background. Coding roles often ask for a coding certification. Always check the job description.
Rules to know: HIPAA and privacy
US health information is protected by a privacy law called HIPAA. Healthcare BPO staff handle sensitive patient data, so companies train employees on privacy, restrict access, and monitor how data is used. Employees are expected to share patient information only when needed for their work and only with authorised people.
